3800 Threw a code- P0401 PLEASE HELP!!!!!

What would cause this?

Also the Ignition volts are at 11.90, and the check battery light went on and off a couple of times and solid for a while but isn't on any more. The volts are still at 11.90max.

Shouldn't the volts be like 13.00 or so and is this the alternator that is failing?

If anyone cares..... I replaced the battery alternator and the tensioner pulley.
I am now running at 14.50 volts so I guess I'm all good there. As far as the EGR code who knows I just cleared it and will wait to see if I throw it again.

For the EGR code, remove the EGR valve and clean out any carbon build up that may have accumulated in it, then re-install it. If it fixes your problem your good, but if you throw the code again its new EGR valve time...
